From 13 August 2007 the forms recommended for use by the Tribunal under the Mental Health (Care and Treatment) (Scotland) Act 2003 (2003 Act) changed from version 5.0 to version 6.0.

Version 5.0 forms are no longer in use.

All forms are available to download and can be viewed, completed and printed using Acrobat Reader v7.

Please note that whilst the Tribunal will do its best to arrange a convenient date and venue for a hearing neither can be guaranteed.
