Request for alteration of hearing type, date, time, or place (civil and CORO cases)

This form should be used for requests to change the type, date, time or place of a hearing under rule 57.  Requests for change which are made close to the date of a hearing should include an explanation of the reason for seeking the change.  The other attendees should be told of the request by the person making it.  If at all possible, the form should set out the views of the other attendees on the request.
